Body After Baby

  • Welcome to Team ClassyFit and CorFit we are happy to bring you onto the team! So lets get started!

    With this program I’ll need the following information from you: Please answer all questions below so your plan can be made specifically for your needs.

  • Front and back in bathing suit or shorts and bra. (Pictures are the best form of seeing your progress)

  • Fitness, weight, strength, cardio. Be detailed on what you would like to achieve during your time on this program. 

  •  Breakfast, Lunch, Dinner, and any snacks?

  • a) Provide nutrition facts on the protein powder you use if any

    b) Current supplements you take to include vitamins

  • a) Much cardio per week

    b) Type of weight training, how many days a week

    c) Time of day you typically workout out

  • Be specific some of your favorite foods may fit your plan, I just need to know what those are.

  • What time you wake up, go to bed, what time you work?

  • If so what gym, this is so I know what workouts to add to your plan by knowing the equipment you have available?

  • Medications, medical history, surgical history, major injuries?

  • Again… excited to have you join our team!! Once I receive this information your diet and workout plan can be started. Thank You for getting this information to me promptly.

    Please be as detailed as possible when filling out this document so that I can tailor your nutrition/workouts to you and your individual goals.
